To download these files from a Macintosh click and hold the mouse button down. When the pop-up menu appears select "Download Link to Disk".

To download these files from a PC right click on the link and select "Save, Download to Disk or Save Link as"

In that case you do not need to fill these forms out again. See the instructions above to determine which documents are required for your particular situation.
